Job Summary:Under the direction of the RN, triages patient phone calls, collaborating with Scheduler to ensure PRN visitsare scheduled when necessary.Coordinates care between practitioners, patients, and field/office staff. Provides effective communication topatients, family members, Agency team members, and other healthcare professionals.Obtains practitioner verbal orders when necessary and transcribes orders into EMR. Receives andtranscribes external orders into EMR.Under the direction of the RN, provides appropriate nursing interventions, provides patient and caregivereducation, and prepares patient and caregiver for discharge from services. Implements infection controlmeasures to protect both the staff and the patient.Performs clinical review of incoming referrals, coordinating with Scheduler on staffing capacity and BranchDirector on potential declined referrals.Communicates with Account Executives, referral partners, patients, and certifying practitioners to obtain allnecessary referral components.Demonstrates commitment and professional growth by participating in Agency-sponsored in-services andmaintaining or improving competency. Participates in the Agency's QAPI program.Promotes the Agency's philosophy and administrative policies.Performs patient visits as needed. Participates in Intake on-call rotation.
